Q: How do I know if I am eligible to take the Student Accident & Sickness Insurance?

A: All students carrying 6 or more credit  hours must participate in the plan unless they “waive” coverage by  August 29, 2009 for the fall term .

Q: Can I get the insurance to cover my spouse and/or children?

A: Yes

Q: What does this insurance cover?

A:  The LMU Student Health Insurance plan covers sickness and injury subject to the plan benefits, exclusions, conditions, and terms.

Q: Does this policy pay all medical bills in full?

A: In most cases it will not cover all bills in full.

Q: Can I go to any doctor?

A:  Yes.  Discounts on fees for services rendered can be obtained by using a First Health provider.  First Health is a nationwide network of providers.  To find a First Health provider go to www.FirstHealth.com

Q: Where do I get a claim form?

A:  A claim form can be obtained on line from the link at the top.

Q: How do I file a claim?

A:  To file a claim, simply follow the instructions on the back of the claim form.

Q: Does this policy cover me for my annual physicals?

A:  Annual Physicals are not covered.

Q: How do I get an I.D. card?

A:  An ID Card will be mailed to you once a list of participating students has been sent to the plan administrator.

Q: What is the policy’s maximum benefit?

A: The Benefit Maximum is $50,000 per sickness or injury per policy year after an annual deductible of $250 has been paid.
